ABSTRACT

Osteoporosis (OP)-related vertebral fractures often occur in the transitional thoracolumbar region,which are the earliest signs of OP deterioration in elderly patients.Such fractures can be regarded as a physical incident that occurs when the vertebral stress load exceeds the mechanical structural strength.To OP patients,preventing the occurrence of OP fracture is the primary goal for preventative measures,treatment and research of OP.This paper reviews the biomechanical research progress of osteoporotic vertebra compressive fracture (OVCF),and advocates early prevention of OVCF in OP patients,who should avoid activities that might exceed the security scope of the vertebral body load in daily life,so as to prevent subsequent fractures of the vertebral fracture cascade.

ABSTRACT

The theory of static and dynamic combination coincides with the effect of osteoblast stress, although there are few articles on molecular biology about the theory in recent years. The treatment of static and dynamic combination theory for fractures and other orthopedic disorders is to use the localized stress fracture, and the stress controls bone formation results through afffecting proliferation of osteoblast differentiation, which has proved by relevant experimental evidence. This article discussed the molecular biology basis about the correlation between theory of static and dynamic combination and osteoblast proliferation and differentiation by stress stimulation.

ABSTRACT

Recent studies have shown that TCM treatment for bone metabolic diseases, such as steroid-induced avascular necrosis of the femoral head, has definite efficacy and has received extensive attention. However, due to lack of molecular biological basis of pharmacodynamics mechanism, it has not yet reached the standard of scientific treatment. The discovery of OPG/RANK/RANKL system has become a new breakthrough point in the prevention and treatment of bone metabolic diseases. The pathogenesis of deficiency of spleen and kidney - blood stasis - caused by phlegm arthralgia caused by blood stasis is closely related to the system. The effect of the final control of the system from the deficiency theory can be expressed through the axial micro-information. This article discussed the TCM syndrome differentiation of steroid-induced osteonecrosis of the femoral head and the relation of regulation of bone metabolism combined with the OPG/RANK/RANKL system, and provided the basis for prevention and treatment of this disease.

ABSTRACT

In recent years, due to the irregular and abused use of glucocorticoid in clinical treatment, the incidence of steroid induced avascular necrosis of femoral head (SANFH) is gradually increasing. TCM for the prevention and treatment of SANFH has received much attention from many scholars. However, due to the lack of the scientific explanation of molecular biology level for its pharmacodynamics mechanism, it is difficult to achieve the purpose of standardized treatment. The discovery of OPG/RANK/RANKL signaling pathway has opened up new shortcuts for the prevention and treatment of bone metabolic diseases. OPG/RANK/RANKL signaling pathway is associated with the pathogenesis of spleen deficiency and phlegm - blood stasis caused by phlegm-blood stasis due to paralysis of SANFH. The treatment efficacy based on the phlegm theory can eventually axial control of the system through the micro-information to express. This article discussed the relevance between the phlegm in the treatment of SANFH and molecular biology mechanism of OPG/RANK/RANKL signal regulation mechanism, and combined the system of bone metabolism regulation mechanism to discuss TCM differentiation of SANFH, with a purpose to provide references for clinical and further study.

ABSTRACT

The spinal manipulation in traditional Chinese medicine (TCM) has a prominent clinical advantage in the treatment of chronic lumbar pain,such as low back pain,but the insufficient basic research on manipulation is a serious obstacle to its development.Using finite element analysis (FEA) can preferably simulate the mechanics under various kinds of spinal manipulation,analyze its action mechanism,test the hypothesis,standardize the operating practices,make quantitative,qualitative and optimal treatment plans,so as to provide an effective method for the basic research on manipulation therapy.This paper reviews the FEA studies of spinal manipulation in recent years,discusses the influence of different spinal manipulation on intervertebral disc,lumbar accessory structures,spinal loads and mechanical stability of the vertebral body.The results show that current FEA studies on spinal manipulation need to make the simulation method standardized and precise.Meanwhile,the research ideas of finite element method should be developed to guide the clinical application of spinal manipulation.
